The Oaks Personal Care Home is designed for residents who desire 24-hour service. All resident rooms are single occupancy and vary in size and decor. An attractively furnished dining area and common space is available to all residents. Although physically attached to The Oaks Nursing Home, The Oaks Personal Care Home is a separate entity and provides a level of care different from that provided in the nursing facility. The most recent inspection of The Oaks Personal Care Home on April 18, 2018 resulted in zero deficiencies. This is the third consecutive deficiency-free survey.
Staff in the personal care home are trained Certified Nursing Assistants and have Registered Nurse oversight, but they do not provide skilled nursing care. Staff are available to assist each resident with their medication regimen, baths, personal care and grooming, as well as prepare meals and provide housekeeping and laundry service.The facility will also arrange emergency transfer of the resident to a hospital if this is ordered by a physician. Each resident is assessed prior to admission to determine if personal care placement is the best level of care for the resident. Should a resident be admitted to the personal care home and over time the resident’s condition changes, priority admission to The Oaks Nursing Home and The Oaks Supportive Care is available.
The cost of services at The Oaks Personal Care Home may be covered by insurance or private payment. Medicare and Medicaid do not assist financially in the services provided at The Oaks Personal Care Home.
